The Cottage Hospital Auxiliary is sponsoring its annual Memory Tree in front of Cottage Hospital on Swiftwater Road in Woodsville, NH. A bulb in memory of a loved one may be purchased for $5 per individual person or $25 per family. Proceeds are used for the Auxiliary's ongoing annual support of Cottage Hospital.

Please print and fill out THIS FORM and mail payment to Cottage Hospital Auxiliary, PO Box 143, Woodsville, NH 03785. Memory Tree Bulb forms are also available at the Welcome Desk at the hospital.

Please join Auxiliary members for the tree lighting and dedication ceremony of the Memory Tree to be held on Tuesday, December 7th at 4:00 PM on the Cottage Hospital grounds. Wearing masks and social-distancing required.
