Services:

The Social Service/Discharge Planning department is available to all patients and their families to aid in dealing with some of the social, financial and emotional challenges often accompanying an illness or injury, that may be identified as barriers for a safe discharge.



Services available include, information about Advance Directives including necessary documents, referrals to community and state agencies for assistance, referrals for medical equipment such as walkers, oxygen and hospital beds for home use when medically necessary, placement in nursing homes or group homes, scheduling family meetings for discharge planning.
