Diabetes is a chronic medical condition which affects children and adults. Management techniques include fostering healthy behaviors, such as losing weight, monitoring blood sugar levels, and meal planning. Diabetes education provides counseling and support based on the American Association of Diabetes Educators (AADE) 7 self-care behaviors. Diabetes Prevention Program

The Prevent T2 lifestyle change program is proven to help people with pre-diabetes and/or at risk for type 2 diabetes make achievable and realistic lifestyle changes and cut their risk of type 2 diabetes by 58%. Prevent T2 is a year-long program with trained lifestyle coaches and a support group. The program meets weekly for the first 6 months, then once or twice a month for the second 6 months.  Learn More >>>
