An Advance Care Directives documents your wishes for care and treatment should the time come when you are unable to speak for yourself due to life-threatening illness or injury. Completing an Advanced Care Directive provides the opportunity to reflect on what you would like to happen; it can also provide the opportunity to discuss your wishes with family, friends, caregivers and your health care providers.
