Please join us for the 20th Annual Cottage Hospital Walk-A-Thon & Road Race! All monies raised will benefit the building revitalization project for the hospital.

Cottage Hospital is committed to providing our community with state of the art equipment and facilities to provide you with the care you expect and deserve. We are proud to have served our com- munity for over 100 years and we look forward to creating an environment that is here for you for a 100 more.

Doing our part...
In an effort to promote healthy exercise, this year the 5K walk registration is FREE! Any money raised is appreciated and will be counted toward various prize levels.

START TIMES:
Walk 9am, Race 9:30am. Race day registration beings at 8am

Course Maps will be available at time of registration, or you may call(603) 747-9189 to have one sent to you ahead of time.

How to Enter:

  1. Pre-Register by filling out this form and returning to the address below or log on and register online.
  2. Bring form and registration fees with you on the day of the race.
    *All sponsorship money will be collected the day of the event.

    Grand Prize - The top two participants who raise $500 or more will get their choice of one of the following overnight stays:
    Lake Morey Resort Fairlee, Vermont, or Woodstock Inn North Woodstock, NH.
    Many amenities included. Top fundraiser will have first choice!
  • Youth (18 & under)
    Grand Prize - $50.00 gift certificate to Wal-Mart
    Runner Up - $25.00 gift certificate to Hubert's

    Grand Prize - Employee Ice Cream Social and plaque (2 Categories: Fewer than 20 employees and 20 or more employees)
